REC GYMNASTICS EVALUATION REQUEST

ASSUMPTION OF RISK

By the very nature of the activity, gymnastics carries a risk of physical injury, no matter how careful the gymnast and the coach are, no matter how many spotters are used, no matter what height is used or what landing equipment, the risk of injuries includes minor injuries such as bruises, and more serious injuries such as broken bones, dislocations and muscle pulls. The risk also includes catastrophic injuries such as permanent paralysis, or even death from landings or falls on the back, neck or head. Gymnastics, or any activity that involves motion, rotation and height in a unique environment, carries with it a reasonable assumption of risk. EVEREST GYMNASTICS TRAINING CENTER INC & H&C INVESTMENTS INC is bound by law to inform all participants of the risk involved in the activity of gymnastics. The parent or caregiver of any child participating in EVEREST GYMNASTICS TRAINING CENTER INC program must sign their acknowledgment below

RELEASE OF LIABILITY

In consideration for EVEREST GYMNASTICS TRAINING CENTER INC & H&C INVESTMENTS INC'S acceptance of the applicant's opportunity to improve gymnastics skills through the use of EVEREST GYMNASTICS TRAINING CENTER INC staff, equipment and facilities, the enrolling student and parent or caregiver realizes the risk of injury involved and hereby agrees to assume the responsibility for his or her own and or child's safety. The parent or caregiver further agrees to save and hold harmless the said facility, its employees and all other concerned and to indemnify them against loss, intending to be legally bound, our signature is offered hereto below to act as our legally binding signature.

MEDICAL EMERGENCIES

GYMNASTICS TRAINING CENTER INC & H&C INVESTMENTS INC and their representatives harmless in the execution of such. Additionally, I hereby agree to individually provide for all medical expenses which may be incurred by me or my child(ren) as result of any injury sustained while participating at or for EVEREST GYMNASTICS TRAINING CENTER INC & H&C INVESTMENTS INC.
